CHOICE OF APPROACHES TO THE TREATMENT OF THORACIC WALL MALIGNANCIES

The paper analyzes results of combined and surgery treatment of 49 patients with primary and secondary thoracic wall malignancies. The choice of the treatment tactics is complicated by the presence of large-sized locally disseminated tumors. For the first time in the clinical practice, the method of selective intra-arterial chemotherapy (SIAC) was applied. Curative effect in the form of either partial regression or stabilization of the tumour growth was observed in 94,4% patients. SIAC was applied as neoadjuvant treatment in 9 operated patients. In 12 patients, it was applied as independent treatment technique. Overall, 31 patients were operated, including 16 patients with the plastic building of the defect in the thoracic wall. It is shown that the wide resection is the priority method in the treatment of malignancies localized in the thoracic wall. Immediate and follow-up results of treatment are evaluated as positive.
